A positive Hoffmann sign does not automatically mean surgery is necessary. This involves two levels. A single pathological sign should be integrated with other pathological signs. For example, Hoffmann sign should be considered together with Babinski sign, hyperreflexia, changes in muscle tone, and so on, rather than standing alone as in this case. In addition, pathological signs are not the only basis. They must be organically combined with symptoms, imaging, functional tests, patient wishes, and other aspects to jointly assess surgical appropriateness. Specifically in this case, imaging did not show spinal canal stenosis or spinal cord degeneration. No functional tests such as electromyography were performed before surgery. The symptoms also lacked extreme features such as unsteady gait, falls, incontinence, or obvious muscle strength decline. Therefore, the necessity for surgery was very weak.
