The primary complaint was chest tightness, yet the physical examination records contain no detailed description addressing this symptom. The diagnosis of cardiac function class III lacks supporting evidence. Preoperative objective indicators such as NT-proBNP were not obtained. Chest tightness has multiple possible causes clinically; this patient had a hiatal hernia, which can also produce chest tightness. The ascending aortic diameter of 53–54 mm exceeded the 50 mm threshold for patients with bicuspid valves and represented the only relatively solid surgical indication. Outpatient notes clearly stated “follow-up with regular re-examination,” yet the physician verbally recommended surgery to the patient. Postoperative improvement in chest tightness is not guaranteed and should have been explicitly discussed beforehand. The agent’s signature on the consent form may reflect understanding and compliance rather than true agreement to proceed. The discharge diagnosis was changed to “status post vascular graft replacement,” with the original cardiac function classification omitted and no formal assessment of functional improvement recorded.
